One requirement outranks everything else an exit device is asked to do. When a room empties out in a hurry, in the dark, nobody should need a key and nobody should need to work anything out. Whatever else that opening is expected to manage comes a distant second.
Call a bar dead and it is usually alive. What has generally happened is that the frame has shifted and the latch is now missing its keeper, or somebody dogged the device down one morning and nobody undogged it, or a vertical rod has grown a quarter of an inch over the years so the two latches release out of step, or the closer has gone lazy and the leaf is no longer arriving fully home. Downtown there is a fourth possibility that belongs to these old rear doors and nowhere else: the opening has been painted around the device so many times that the pad runs out of travel before the latch has cleared.
